Please be aware that completing this form does not guarantee acceptance into the Chef to Plate program. GIG reserves the right to evaluate and determine eligibility based on the information provided.
By completing this form, you consent to be considered for inclusion in our program, allowing GIG to showcase your establishment on our website and in our marketing materials.
This agreement is valid for 12 months from the issuance of the Participation Certificate. Should changes be made to your gluten-free menu or practices, please notify GIG promptly. We may conduct a secret shopper visit to ensure the continued quality of our program.
